As a new patient, you will need to fill out a short medical form before your first appointment. You can send it by email, or print it and bring it to the clinic. This information helps us open your file and understand your health, including your history and the medications you take.

04 Insurance
In most cases, we will send the documents to your insurer. Depending on your company, you may have to pay the fees before being reimbursed. Our team will provide the documents you need. Note that over-billing an insurer is prohibited; make sure you know the terms of your policy.
